I have professional experience working at the management level in assisted living/memory care assisted living facilities, skilled nursing/short term rehab facilities, and home care companies. I have experience working with people on hospice and those living with dementia/Alzheimers. I have been working with and for the elderly and their families and professional caregivers for 10+ years. The main service I provide is care management- helping clients manage their medical and caregiving needs, providing assessments of clients needs, providing transportation advocacy and note taking in Dr appts and managing the scheduling of Dr appts, connecting clients and families to resources such as elder law attorneys specialized drs or support groups, helping clients find the right home care or tour assisted living or long term care facilities, etc. I am unable to accept jobs less than $35/hr.
