My decade of experience as a Registered Nurse Case Manager has fueled my passion for supporting the independence and well-being of seniors. I offer professional, detail-oriented care, including medical management, which consists of coordination of post acute care and chronic care for patients who are living in home or being discharged to facilities. I collaborate with physicians to ensure safe and timely discharges, review clinical documentation and determine appropriate levels of care, discharge plans, and service expectations. I monitor patients progress after discharge, life changing events, or general debility and fully address barriers to care. I offer medication set ups management and reminders, as well as transportation to and from doctor's appointments.
