About Iris

I am a compassionate and dedicated healthcare professional with over 16 years of experience as a Certified Nursing Assistant and 5 years of experience as a Care Navigator specializing in diabetes care and preventive screenings. I have worked in a variety of healthcare settings including long-term care, rehabilitation, hospice, dementia care, surgical units, ICU support, and in-home patient care. I am bilingual in English and Spanish and passionate about providing high-quality, patient-centered care to individuals from diverse backgrounds. My experience includes patient education, vital signs, diabetic screenings, EKGs, preventive health services, scheduling, and supporting patients and families through all stages of care.
