Stroke patient with global aphasia, but he can answer yes, no and some questions with accuracy. Speech, occupational and physical therapy and home health nurses come to see him several times per week, & one of his sons lives with him-so family is available to help and all stop by several times a day to check on him. His daughter organizes all medications and has detailed instructions for them (only gives meds two times each day). He does weigh 220ish pounds, and his right side is weak after the stroke, so he needs support each time he stands or walks (usually just stands and gets in his wheelchair).
We have a video monitor for when he's sleeping, bed alarm and bed rail to keep him safe, & nurse call buttons in his room & throughout the house. His restroom has been made handicap accessible, so everything is set up for safety.
Services Needed: Medication Prompting, Light Housekeeping, Bathing / Dressing, Meal Preparation.
