Looking For (an Entering) Middle School Math Tutor

My fifth-grade daughter has some major holes in her math comprehension. She has dyslexia and ADHD so I'd like to find someone who has experience teaching children with special needs. Please tell me how you would help her make progress over the summer. Specifically, how would you identify her weaknesses, how many hours would you want to work with her a week and what is your rate per hour. I would prefer to find a library or alternative location for you to work with her at. I would like to find someone who could work with her in person several times a week during the summer (flexible to your schedule) and then continue maybe on a weekly or virtual basis during the school year.
