We care for those patients diagnosed with a terminal illness who would like to stay home near their loved one, pain free, with dignity. Hospice patients are comforted & cared for when they can no longer be cured. Lifeway Hospice offers an interdisciplinary team for symptom management, social work for long term planning, advanced directives & assistance with Medicaid & VA benefits. We offerchaplain, counselors, & bereavement for patient & family support. Nursing visits to assist with medication management. CNA visit to assist with personal care. MD & NP visits in home. When appropriate, patients have the availability of PT/OT/ST, Music Therapy, Aqua Therapy, Recreational Therapy, Massage Therapy, Pet Therapy & Volunteers.
