I also offer a companionship service to the elderly, this could be for help at home, form filling, days out, joining a club or group, help with shopping, take up a hobby or just have a cup of coffee and a chat. I have clinical skills including catheter care, stoma care & ventilation knowledge & machine maintainence. I offer a 'take a break service' to give family members & main carers a break during the day & I can also offer overnight care. | I am an excellent communicator, conscientious, and I am able to remain calm under pressure. I am known to show good professionalism at all times and am aware that confidentiality is a priority. I have experience of completing daily reports, updating case notes, patient discharge summaries, conducting annual review meetings with relevant agencies, risk assessments, agreeing short and long term goals, person- centred support plans and initial assessments in the persons own home. Whilst at Parchment Trust I organised many group activities, social gatherings, fund raising events and day trips. I also supported groups to go on holiday. Some of my successes included organising a Pirate Walk fund raiser and building a Santa Sleigh to be displayed in the Asda supermarket.
