| Experience in dementia, reablement, learning disability and supporting people to be independent as much as they can be. All required training are up-to-date with DBS check. From previous work experience was a home care Supervisor and was responsible for assessing the needs of people, setting up the care plans for each person and making sure we deliver person-centred care. Also, build trusting relationships with people, their families, staff and other professionals such as district nurses, GPs and occupational therapists, assess and order basic moving and handling equipment for people and putting moving and handling risk assessment in place for them.
