We will help you and your family by providing you both with the following services:
24 hour on call support from our nurses.
Assistance with activities of daily living (personal care, meal preparation,housekeeping).<br/>Live in care for as many days as needed.<br/>Case management.<br/>Palliative care.<br/>Respite care. <br/>Hospital support (providing a care giving or guiding you through the hospital system).<br/><br/>Our nurse creates, manages and oversees each client's individual care plan. This allows consistency in the client's care. You can feel comfortable with you loved one's care knowing that a nurse will be supervising and will contact you if there are any changes in your loved one's condition. <br/>Our caregivers have all been hand selected by our Director of Nursing. They go through extensive, interviews, training, and background checks. We have hired local caregivers to ensure dependability to our clients. Our nurse determines which caregiver she feels will best match the client's personality and needs after the first initial assessment.<br/>To keep your loved one happy, healthy and safe at home, we will provide the following services:<br/>Reduce stress and anxiety in the home by assisting the family with creating a structured care plan.<br/>Act as an advocate: a support system between the client and the physician.
