We specialize in post rehab and post hospital stay especially for those with movement disorders, Parkinson's, PSP, strokes and generalized muscle deconditioning.
Our services fall under 3 categories:
#1. CARE-GIVING: services are provided by home health aides, companions and certified home health aides.
#2. Chronic Illness & Symptom Management: These services are designed to reduce frequent trips to the hospital and rehab facilities.
The care programs are designed by nurses and geriatric care specialist.
#3. CARE MANAGEMENT & CARE COORDINATION: These services are provided by geriatric care managers. The goal is to make long distance care giving easier and more gratifying for the long distance or the very busy family caregiver.
