OUR MISSION:

We Are Family Home Care strives to foster independence for our clients and peace of mind for their families by building relationships with compassion, communication, and trust. We believe providing care and assistance in the familiar environment of the home enhances quality of life for seniors, people with disabilities, and others coping with severe illness and/or recovering from surgery.

OUR CAREGIVERS:

Our Caregivers are the backbone of our success and represent our family while assisting yours. Each Caregiver is an employee of our company and not an independent contractor. The responsibility to ensure quality care is ours to give you and your family peace of mind and protection for you or your loved one. We are licensed by the Indiana State Department of Health, bonded, and insured. All Caregivers go through required background checks before they are hired and are required to be tuberculosis (TB) tested.

We offer a complete range of services, which we can customize to suit each individual client and situation.

One of our Care Coordinators will schedule an initial consultation with you. Then we'll carefully develop a unique Client Care Service Plan. Once both the Care Coordinator and the client (or authorized family member) have agreed upon and signed this plan, our Caregivers will use it as a guide as they assist clients with daily living activities.

We Are Family Home Care's policy is to re-evaluate the Client Care Service Plan at least every 4 months, in order to address any changes in the client s situation. If warranted, we may re-evaluate it more frequently, if we've determined that a client's needs have changed.

We welcome and solicit input from our clients and their families in order to provide the best possible service.

Our Caregiver Services consist of the following:
*COMPANION/HOMEMAKER CARE
*ATTENDANT/PERSONAL CARE.

Our Caregivers can provide hourly services, 24-hour continual care, or live-in services.

By combining aspects of our Companion/Homemaker and Attendant/Personal Care services listed above, we can accommodate any of the following needs:
*SENIOR CARE
*ALZHEIMER'S/DEMENTIA CARE
*RECOVERY CARE (post-surgical, serious illness)
*RESPITE CARE
*HOSPICE CARE SUPPORT (end-of-life care)
