Synergy is a national non-medical home care company that was established in 2002 and franchised in 2004. We have over 250 offices nationwide.
We excel in responsiveness, processes and matching caregivers to clients based on ourcomplimentary Home Assessment. Hospital sitting, move in/settle and holiday support are also available. Please contact us if you need home care for individuals with Alzheimer's, Multiple Sclerosis, Arthritis, Diabetes, Stroke Recovery, ALS, Memory Care, Parkinson's or Orthopedic Recovery. We work with local families and with family members who live outside Santa Cruz County and desire to arrange care for local loved ones. <br/> <br/>How we work with you and your doctors: <br/> <br/>- Work with you to create a personalized Care Plan <br/>- Integrate the discharge plan from your doctor or rehabilitation facility <br/>- Conduct a complimentary safety assessment of your home <br/>- Communicate with you on a regular basis, and give you Care Notes each week <br/>- Conduct Quality Checks at the clients' homes while caregivers are on site <br/>- Provide a telephony system for caregiver check-in and check-out <br/> <br/>We have synergies in the community: <br/> <br/>We are members of the community and well connected with the partners in the senior care ecosystem.
