I am a Registered Nurse, and Rehabilitation Consultant. I have been in the medical field for over 18 years. I specialize in medical case management. I offer case management services. I provide care to elderly people or anyone wanting to stay in the home as long as possible but has some needs that require assistance. I assist with home care needs, and update family and providers as requested. I attend medical appointments and coordinate care, and update family/TCU's or nursing home, and the providers on treatment plan and ongoing needs. I would love to help you or your loved one, advocate for them, and make sure they are receiving excellent ongoing medical cares and their quality of life is maintained as they go through any disease or injury process.
