Gracious Heart is an Adult Family Home facility located in Des Moines, WA. We offer dignified senior care 24 hours a day, seven days a week. Our care services are founded on the belief that you or your loved one's safety, independence, and privacy are equally important to the quality of care they receive. Each resident has an individualized care plan as unique as they are and tailored to their specific needs and preferences. Our personalized, round-the-clock care allows you or your loved one to be as self-sufficient as possible while receiving the necessary personal assistance whenever they need additional support. The Gracious Heart Adult Family Home team includes well-trained, credentialed, and licensed caregivers to help manage care and chronic conditions such as diabetes, COPD, hypertension, dementia, and health challenges. Our caregivers are licensed Home Care Aides (HCA) or Certified Nursing Assistants (CNA). Every staff is also certified in CPR, First Aid, Nurse Delegation, Mental Health and Dementia specialty training, Food Handling, and receives 12 hours of Continuing Education annually.
