Skilled Nursing Services (SNS)
For persons who have chronic/serious illnesses, care is provided to ensure improvement in the patient's condition. Treatments may include:
Wound care with vacuum assisted therapy
Medication administration, education and management
Administering of IV drugs
Tube feeding
Monitoring of vitals, including blood pressure, weight, blood sugar, etc.
Personal Support Services (PSS or PSSX)
For clients in need of daily living assistance for basic home duties and chores, and can include:
Bathing & grooming assistance
Meal preparations and/or assistance with feeding
Light housekeeping
Running errands
Accompaniment to doctor's visits & other appointments
Community Living Support (CLS)
Primarily designated for persons with developmental/physical disabilities, to promote personal independence as members of the community and as active individuals in their homes. Activities can include:
Assistance with money management
Weekly community engagement activities
Assistance with self-medicating
Daily chore reminders and oversight
Communication building
Community Residential Alternative (CRA)
This type of care is for persons in our community group homes, where they will live with at least (3) other residents at a time. Individual plans can include:
Assistance and/or training in activities of daily living, such as grooming, personal hygiene, feeding, toileting, transferring, etc.
Assistance with basic household duties, such as meal preparation, laundry, bed-making, housecleaning, etc.
Diagnosis-related services, such as accompaniment to medical appointments, assisting with therapeutic exercises, and assisting with or supervising self-administration of medication
Taking individuals for medical care, therapies, personal shopping, recreation and other community activities
Development of interpersonal skills
Community Access Individual (CAI)
We can also help individuals obtain and maintain socialization and adaptation skills to help them independently function outside of the home. Individual plans are similar to (CRA), and can also include:
Assistance and/or training in activities of daily living, such as grooming, personal hygiene, feeding, toileting, transferring, etc.
Assistance with basic household duties, such as meal preparation, laundry, bed-making, housecleaning, etc.
Taking individuals for medical care, therapies, personal shopping, recreation and other community activities
Development of interpersonal skills
Environmental Accessibility Adaptation (EAA)
For persons in need of home modifications to help with safely completing activities in their homes. This can include:
Stair lifts
Wheelchair ramps
Specialized bath safety modifications
Specialized Medical Equipment (SME)
We are also able to provide equiplent for those with physical disabilities, to assist with daily functions:
Hoyer lifts/Stand-Up lifts
Wheelchairs/Walkers
Hospital beds
Gait belts
Oxygen machines
Ventilators & Respiratory supplies, etc.
Specialized Medical Supplies (SMS)
These bulk supplies are designated for those suffering from chronic/serious illnesses, and can include:
Adult diapers
Gauze dressing/bandages
Sterilization products
Feeding tubes
Specialized latex gloves, etc.
