We are a home care registry that ONLY refers full-time, trained private-duty live-in care providers who furnish non-medical home support services. We help individuals stay safely in their homes and retirement communities. EldersChoice provides geriatric case management and care coordination to promote client choice and preservation of self-determination. This is why we are called "EldersChoice".

Geriatric case management and care coordination is provided in Connecticut to:

- Assist the client/family to access the appropriate community services,
- Collaborate non-medical needs with medical, home health, palliative, and hospice providers; pharmacies; and, durable medical equipment companies
- Case managers to attend medical appointments and visits to clients in hospitals and rehabilitation centers as needed
- Regular home visits by EldersChoice
