Conducting care-planning assessments to identify needs, problems and eligibility for assistance; Screening, arranging, and monitoring in-home help and other services. Offering a free one-hour consult with Advanced-Practice nurse or Medical Director (MD) as needed.

Assistance with financial, legal, or medical issues. CO.ComplexCareManagmentandWellness (CCCMW) offers a multi-disciplinary team-based approach to assist Elderly and those with complex medical conditions with assistance from specialists to improve the overall wellness and coordination of care for our clients. Our services avoid future problems and ensure to conserve assets, provides crisis intervention and mitigation. Providing client and family education and advocacy.

The owner of this company has been in Nursing for over 30 years and specializes in Functional Nutrition (Certified), Neurology, Alzheimers and Dementia, as well as being board certified in Hospice/Palliative Care. Our mission is to provide the most optimal care for persons with advanced illness with the "whole person" concept as our objective in goals of care.

We take on clients as if family; go on many special outings and make each and every client experience truly special and unique. We regularly take our clients out to eat, participate in physical activities as tolerated, provide non-denominational spiritual support, and social services as needed.

Experience with PT/OT, Art and Pet Therapy. Care plans are created specifically for each individual.

All providers are English speaking, US Natives, Bachelor prepared, non-smokers, Pet and Children lovers, and eager to improve the quality of lives of those we serve.
