About who needs care: 1) Alzheimer's - dementia (moderate) - able to perform most ADL's with encouragement. Needs help with standing and sitting. needs prompting and direction though will feed himself, brush his teeth and use bathroom by himself. He is incontinent of urine only-leaks - will wear adult diapers- diapers changes in morning/evening. will need help with putting socks/shoes on and transferring in and out of bed or chairs. other medical hx - Asthma, HTN, HLD. Will not need help with medications.
2) board games, short walks
3) wake up at 7am, takes medications, breakfast, shower (supervised), short walk, board games, lunch, tv (westerns), reminiscing talks - old job-architecture + talks about the past, some physical exercises - sitting/standing. bed by 9:30 p.m.
About the care needs: 1) Compassionate, patient, tidy, proactive
2) no specialized skills
3) no specific hobbies or interest.
Services needed include: feeding, bathing / dressing, and mobility assistance.
