If you are passionate about helping others and making a difference in your community, becoming a mentor might be for you. Becoming a mentor gives you the opportunity to host an adult with disabilities in your home. The independent providers (mentors) will provide guidance, support, and care for the individual living in your home. You will be working with the mentee on goals which could include things like, shopping, budgeting coping skills, cooking meals, personal hygiene, and assisting them in ways to integrate into their community. You will be an advocate/mentor helping them to live their best life possible. This partnership allows you to establish a meaningful relationship with the person you are hosting as you invite them to become part of your family. The mentor is an independent contractor not an employee of LSI. This position requires an understanding of intellectual disabilities and mental health issues and a commitment to supporting this person to have their best life possible. Host Home Independent Provider's (Mentor) Minimum Requirements: Must be at least 21 years old with a GED or a HS Diploma Must possess a valid driver's license Must pass criminal background checks, MVR checks and sex abuse registry checks Must be able to meet the needs of the mentee Must have reliable transportation Must be responsible, reliable, dependable and motivated to help this person live their best life Must have a passion for serving others Must have effective communication skills, be patient, positive, and understanding of intellectual disabilities and mental health issues Must understand the crisis cycle and positive behavior supports Must be a strong advocate Must be able to respond to emergencies or to otherwise meet the needs of the mentee as they arise. Essential Duties and Responsibilities (including but not limited to): Meet all contractual obligations including completing required training, home safety check, have an appropriate bedroom and other requirements for mentee placement Responsible for accurate and timely completion of all required documentation and forms, this includes monthly documentation of services and census forms, and disruption of care forms when appropriate Maintain knowledge of person's care plan and goals and actively work towards those goals and communicate any needed changes Willingness to have Host Home Program Coordinator (HHC) complete home visits (scheduled and unscheduled, as needed) to ensure well being of person served Communicate all concerns to HHC and Care Team
