About who needs care: heart condition, dementia, has been taken off all medications by cardiologist, neurologist and pcp. given about a year to live and wants to do it at home About the care needs: she is on hospice so a nurse comes 2x a week to shower her. she has another nurse who checks vitals weekly and a social worker that checks in. we need someone in the evening to transition her to bed after dinner and get her to go to sleep which tends to be the most difficult time of day for her. Services needed include: companionship.
