Two choices for this type of care are 24/7 in-home care and live-in care.

While similar sounding, these are two are different models of care. “The key distinction between 24/7 care and live-in care is that 24/7 care speaks to availability,” notes Ann Collette, chief strategy and marketing officer of Masonicare, which provides residential, health and ancillary care to older adults. The structure provides more hours of care, she explains, but doesn’t “necessarily affect the intensity of services.”

Aren’t sure which is best for you or your loved one? The difference is staffing and supervision level, not necessarily the intensity of services — 24/7 uses rotating shifts so someone is always alert, while live-in prioritizes continuity and companionship.

Match the model to clinical needs and nighttime risk. 24/7 care fits higher medical or safety needs (wandering, frequent repositioning, post-hospital recovery), while live-in care works well for medically stable seniors who need daily help and occasional overnight support.

Live-in care is typically more cost-effective (often ~$8K–$12K/month), while fully awake 24/7 coverage can run ~$15K–$25K+ because multiple caregivers are staffing the home; local hourly baselines (about $22.54/hr on Care.com) and agency models shape the final price.

What is 24/7 home care?

When people hear “24/7 care,” they often picture one specific scenario, notes Collette, when in reality, it is a much broader concept. “The term simply means support is available around the clock, but the structure, intensity and level of clinical oversight can look very different depending on the setting and the person’s needs,” she explains.

In a home setting, this generally means that a caregiver is “awake, present and responsible at all times, including nights, weekends and holidays, not just during scheduled visits,” explains Bryan Dylewski, founder of Solenvia, a senior home care franchise. He adds: “What that support includes depends entirely on the individual.”

“The 24/7 care model involves multiple caregivers rotating shifts to ensure uninterrupted supervision and assistance day and night,” Dylewski continues.

“Choosing the right [care] model requires an honest assessment of medical needs, functional abilities, safety considerations and the role family can realistically play.”
— Ann Collette, residential, health and ancillary care expert

What is live-in care?

With live-in care, older adults remain at home with one dedicated caregiver who resides in the home for several consecutive days.

“The caregiver provides hands-on assistance with daily living activities, supervision and companionship,” explains Dylewski. However, he adds, live-in care does not mean the caregiver is working 24 hours straight.

“Caregivers are legally entitled to sleep time and breaks,” Dylewski explains. “Typically, services are structured around a set number of working hours per day, with rest periods factored in.”

Similar to other types of in-home senior care, both live-in care and 24/7 care can be provided by different caregivers, based on needs, including home health aides and skilled nursing assistants.

The former, Colette explains, involve licensed nurses being duty at all times. “This is best for individuals recovering from hospitalization or managing serious health conditions that require continuous clinical monitoring,” she notes. “This may include IV therapy, complex wound care, ventilator support or intensive rehabilitation.”

Who is each type of care ideal for?

Aren’t sure which model makes the most sense for you or a loved one. The most best thing you can do, Collette notes, is assess the type of care needed. “Choosing the right model requires an honest assessment of medical needs, functional abilities, safety considerations and the role family can realistically play,” she explains.

That being said, here’s a quick breakdown of both.

24/7 in-home care

This type of care, Dylewski notes, is best for families who need the peace of mind that someone is actively monitoring their loved one at all times. It’s especially important for seniors with higher care needs, safety concerns or frequent nighttime needs, he adds.

24/7 care is also best for:

Seniors who want to remain in their own home but require consistent, full-time care.

Seniors who may wander or become disoriented.

Individuals who need assistance throughout the night or frequent repositioning.

Seniors recovering from surgery or a hospital stay.

Those with mobility issues or chronic conditions requiring close attention.

Live-in care

“The live-in care model works well for someone who is medically stable but needs consistent supervision and assistance with daily activities,” says Collette. “If care needs become more clinical, such as wound care or injections, a licensed nurse can be layered in to provide skilled services while the caregiver continues day-to-day support.”

Live-in care is often best for:

Seniors/families who value continuity and consistency, as one primary caregiver builds a strong relationship with the client.

Individuals who need consistent, hands-on assistance with daily activities.

Seniors who want companionship and stability.

Individuals with mobility limitations.

Seniors who may need occasional nighttime support, but not active supervision all night.

“Live-in care is often more cost-effective for seniors who do not require continuous overnight supervision, and it can be a cost-effective alternative to nursing homes or assisted living.”
— Bryan Dylewski, home care expert

What are the biggest differences between 24/7 and live-in care?

The biggest differences between 24/7 care and live-in care are staffing structure, supervision level and cost, explains Dylewski.

“For 24/7 care, multiple caregivers rotate shifts to ensure someone is always awake and actively working,” he continues. “For live-in care, one caregiver resides in the home but has breaks and sleep time factored in.”

How much does each type of care cost?

Because 24/7 care requires multiple caregivers working different shifts, it is typically more expensive than live-in care, notes Dylewski. “Live-in care is often more cost-effective for seniors who do not require continuous overnight supervision, and it can be a cost-effective alternative to nursing homes or assisted living.”

“A live-in caregiver model can range roughly from $8,000 to $12,000 a month depending on geography and agency structure,” notes Collette. “If a family requires fully awake, rotating shift coverage so that someone is alert every hour of the day, costs often climb to $15,000 to $25,000 per month or more.”

The reason, she explains, is because you’re essentially staffing one individual around the clock. It is highly personalized, but it is also highly labor intensive.

Colette also notes that, when it comes to 24/7 care, there’s not a single product/pricetag model. “It is a spectrum,” she says. “The real question is not just ‘What does it cost?’ but ‘What level of support is clinically appropriate and financially sustainable?’”

Live-in caregivers generally charge by the hour, with different duties and locations affecting rate.

According to Care’s Cost of Care Calculator, in-home caregivers in the U.S. are currently asking for an average starting rate of $22.54 an hour, but here are posted rates in top cities.

Current posted in-home caregiver rates in top U.S. cities*

City, State

Hourly In-Home Care Rate

Atlanta, Georgia

$21.61/hr

Brooklyn, New York

$23.70/hr

Charlotte, North Carolina

$21.46/hr

Chicago, Illinois

$22.84/hr

Denver, Colorado

$25.77/hr

Houston, Texas

$19.38/hr

Miami, Florida

$20.04/hr

Orlando, Florida

$20.28/hr

Philadelphia, Pennsylvania

$21.22/hr

Phoenix, Arizona

$23.64/hr

Portland, Oregon

$25.70/hr

San Antonio, Texas

$19.62/hr

San Diego, California

$26.11/hr

Seattle, Washington

$28.84/hr

Washington, D.C.

$22.50/hr

*Based on average posted starting rates from in-home senior caregivers listed on Care.com, as of March 9, 2026.

The bottom line on 24/7 vs. live-in home care

Choosing between 24/7 and live-in care ultimately comes down to how much care you or your loved one needs, as well as your budget.

“The most responsible approach is aligning cost, clinical need, and quality of life from the start,”
