We have over 10 years of business experience and our CEO has been a geriatric nurse for over 30 years. Our areas of expertise include Senior Wound Care, Post-Hospitalization Care, Dementia, Diabetes, and Hypertension. We also specialize in caring for Veterans.
Our services range from Companionship, Personal care, Cleaning, Errands, Doctors Appointments, Medication Administration and Reminders, Ambulation transfer, Skilled Nursing, IV Therapy, TPN, Tube Feeding, Wound Care, Wound Vac. Management, Ventilator Care, Central Line Care, Lab Work, Tracheostomy Care, Live in or Live out, and many others.
In addition to these services, we offer special programs that improve the lives and care outcomes of all our patients. These include:
-Homecare Financial Advising Program- Our care advisors are trained to help families locate national, state, local and non-profit financial assistance programs that help save families thousands of dollars in out of pocket costs.
-Hospital Re-Admission Prevention Program- Our staff is taught to monitor vitals, treatment plans and medication adherence in order to prevent your loved one from going back to the hospital.
-Fall Prevention Program- Our staff is taught to locate fall hazards as well as closely monitor your loved one's mobility so that we can actively plan and prevent your loved one from falling.
-Suicide/PTSD Surveillance Program- Our staff is taught to recognize the warning signs of suicide and post-traumatic stress syndrome and how best to address these conditions. This program is specially tailored for veterans and patients who have experienced traumatic circumstances.
-Cognitive Engagement Program- Our staff is taught how to engage clients in meaningful activities that help to improve cognition, memory, and socialization skills. This program is specially tailored for patients with Dementia and Alzheimers.
