This holistic approach to care allows us to customize a care plan and service plan tailored to the needs of our clients and their family. Shortening their hospital stays which will lessen the chances of hospital borne infections and hospital readmission, It lays the foundation to customize a plan of care that addresses the skilled nursing needs, personal care needs and homemaker needs of our clients in the comfort of wherever they call home.
Our Personal Care Aides, Administrative Team Members and our Nursing Team all receive additional training, backed and certified as an (Associate Member of the NCCDP National Council of Certified Dementia Practitioners) and by (In The Know Caregiver Training).
Our training model is exclusive to the Loving Hearts Standard of Excellence and in accordance with the Georgia Department of Community Health.
Our Licensed Practical Nurse and Registered Nurse team collaborates with our Consulting Pharmacist to ensure the best communication when partnering with the client's medical providers. By taking these measures, Loving Hearts is able to ensure that our clients are receiving consistent and professional in-home care.
