I have performed total patient care for end-stage Parkinson's, Alzheimer's, and dementia patients; which included medication, personal hygiene, range of motion and tube feeding, transportation to doctor's and rehabilitation appointments, and running errands. I determined needs in initial home evaluation and initiated beneficial changes. I communicated well with doctors, colleagues, and patient families, ensuring continuity of patient care.
