I work directly spiritually, emotionally, physically, and psychologically with the patients, their families and the hospital staff. I observe the patients' sound judgment, spiritual assessment and physical conditions as well as applying safety measures as necessary depending on the patient and the circumstances.
Summer 2009, 2010 and 2011 Chicago, IL- Elderly care; spiritually, emotionally, physically and psychologically. My services included:
* Providing medication, food, and drinks and making sure that we meet all the doctor's appointments.
* Spiritual care and church services.

Position: Assistant Case Manager. 2005-2007
* Provided ongoing counseling services at Mityana Rehabilitation Center, a residential treatment facility for young men and women who were referred from the Department of Youth Services, Department of Social Services or the Department of Mental Health. Clients of Mityana area and other towns (and their families) experienced a variety of complex psychiatric, medical, and social problems.
* Offered focused counseling services on human growth and development throughout a broad range of personal, familial, vocational, and interpersonal contexts.
* Assess clients' eligibility and need for various federal funded programs and make determinations regarding appropriate case disposition and required referrals to community resources.
* Assist applicants in obtaining access to public benefits or other community or volunteer services by making referrals and assisting with federal and county applications as needed.
* Participate in case conferences, problem solving and discharge planning meetings at hospitals, nursing facilities, and rehabilitation hospitals.
* Process intake applications for services and determined eligibility by conducting interviews.
